Thank you for selecting our office for your oral surgery needs.


    We strive to do our best to provide the best possible care to you. If you have dental/medical insurance we are willing to submit to your insurance carrier for payment. Please note that most services rendered in our office are considered dental in nature but please bring medical information with you as some procedures are considered medical and will require being submitted to your medical carrier. Please read carefully the information below concerning our financial procedures.


    Payment is expected at the time of your visit for all services rendered if you don’t have insurance. If you visit is an emergency and you require immediate attention, payment arrangements will be discussed with the Office manager and it may be necessary for you to sign a financial agreement.


    If you have insurance, a 30% down payment of the fee is required plus any deductible amount that has not been met at the time of the surgery. If you are unsure if you’ve met your deductible we can call your insurance carrier to verify this information. Any amount not covered by the insurance company will be your responsibility. We will send you a copy of the explanation of benefits form you insurance carrier when we send you the bill for the balance.


We accept cash, credit cards payments, however, for patients who pay be check, please be advised that there is a $30.00 charge for returned checks.

    The adult who accompanies a minor to our office for treatment is responsible for payment of services rendered.


    If my delinquent account is sent to a collection agency, I agree to the addition of a collection fee of $50.00 or 20% of the balance owed, which ever is greater.

       

    For our patients who are referred to our office for treatment by an attorney or Workman’s Compensation for PIP cases, we will need verification from the attorney or your place of employment as to where to submit your bill for payment and we will also need proof that you have filed a claim with your PIP carrier.
